This document describes the policy of ANTSA Psychology (Sally-Anne McCormack) for the management of the practice’s clients’ information. The psychological service provided is bound by the legal requirements of the Australian Privacy Principles (2014) (APP) and the Victorian Health Privacy Principles (HPP).
Client files are held in a secure cabinet which is accessible only to authorised employees. The information on each file includes personal information such as name, address, contact phone numbers and other information which is relevant to the psychological service being provided. Contact details are also kept on a computer system and saved to a server which is located at the psychologist’s primary practice.
The information is gathered as part of the assessment, diagnosis and treatment of the client and is seen only by the psychologist. The information is retained in order to document what happens during sessions and to assist with treatment. In addition, information needs to be retained to meet the legal requirements of the APP and HPP. No personal or health information is ever provided to any overseas entity.
Clients may request to see information about them kept on file. The psychologist may discuss the contents with them or provide a copy. All requests for access to information need to be lodged with ANTSA Psychology (Sally-Anne McCormack). These requests will be responded to within 14 days and an appointment will be made if necessary for clarification purposes.
If you have a concern about the management of your personal information, please inform Sally-Anne McCormack. Upon request, you can obtain a copy of the Australian Privacy Principles, which describe your rights and how your information should be handled. Ultimately, if you wish to lodge a formal complaint about the use of, or access to, your personal information, you may do so with either the Office of the Australian Information Commissioner on 1300 363 992 or the Victorian Health Services Commissioner on 1300 582 113.
